There's also a second trap: describing the output instead of the outcome. "Redesigned the checkout flow" is not an accomplishment. Don't Have Numbers?
Design impact is often indirect, and that's okay to name explicitly. Not every accomplishment needs a conversion metric. Qualitative signals count: stakeholder feedback, number of revision rounds eliminated, reduction in support tickets, time saved in the design process, or the avoidance of a costly build. "Caught a critical navigation failure before launch that would have affected 100% of users" is a strong accomplishment even without a revenue number attached. Frame qualitative impact as a risk mitigated or a cost avoided.
Match the Level
Junior and mid-level designers should focus on craft quality, user research thoroughness, and the accuracy of their execution. Senior designers should emphasize decisions made, influence on product direction, and systems built for others. Staff and principal designers should document org-level impact: how the design system scaled, how the research practice was elevated, how other designers leveled up because of you. The higher the level, the more your accomplishments should describe leverage — things you set in motion that multiplied impact across the team.
